Many Indians believe that paying health insurance premiums on time ensures smooth claim settlement. However, insurers often reject claims due to specific rules and conditions. Understanding these reasons can help policyholders avoid disappointment during medical emergencies.

One common reason is non-disclosure of medical history. If a person hides or forgets to mention pre-existing illnesses, insurers may reject claims when hospital records reveal them. Another issue is the waiting period. Even disclosed conditions like diabetes or hypertension may not be covered until the waiting period, often two to four years, is completed.

Policies also have exclusions. Treatments such as cosmetic surgery, experimental procedures, or routine outpatient visits are usually not covered unless specifically mentioned. Claims may also fail if medical necessity is not proven. Insurers check whether hospitalisation or procedures were essential according to accepted medical standards.

Incomplete paperwork is another hurdle. Missing bills, discharge summaries, or incorrect medical records can lead to rejection. Finally, cashless claim denials occur when hospitals refuse cashless requests. This does not mean the claim is invalid. Policyholders can pay upfront and later apply for reimbursement with proper documents.

Experts advise buyers to read policy terms carefully. Premium cost should not be the only factor. Inclusions, exclusions, waiting periods, co-pay clauses, and hospital network conditions must be understood before purchase. Awareness of these rules ensures smoother claim settlement and reduces stress during emergencies.
